A guide to Bakunahan sa Diliman

The UP Diliman (UPD) COVID-19 Task Force (TF) recently created a handy online guide to help those scheduled for COVID-19 vaccination to prepare for their scheduled day at the Bakunahan sa Diliman in the College of Human Kinetics (CHK) gymnasium.

Bakunahan sa Diliman is a project of UPD and the Quezon City local government to open a vaccination site at the CHK gymnasium for registered and confirmed UPD personnel and select Barangay UP Campus residents.

In the past few months, the TF conducted several rounds of vaccination pre-registration and confirmation through email.

Only those who have received a notice of their vaccination schedule for that particular day and have complete documentary requirements may go to the CHK site.  Walk-ins will not be accommodated.  Read here for more information on registration and scheduling.

There will be forms to be filled at the vaccination center: the Health Declaration and Screening Form, the Sinovac Consent Form, the top part of the Validation Form and the Vaccination Card. Bakunahan sa Diliman is open to UPD faculty, REPS and administrative staff; professors emeriti and their spouses or significant other; newly retired faculty and staff (starting January 2019); UPD employees under job order (JO), contract of service (COS) and non-up contractual arrangements; UPD’s custodial workers and security guards; and UP System employees working at UPD (regular and contractual/COS/JO).

On one’s scheduled day, he/she is advised to bring the medical documentary requirements (when applicable), two pens for personal use and drinking water.  One may also want to bring a light snack (crackers or biscuit), a hand fan or pamaypay (electric/otherwise) and maintenance medications.  Of course, do not forget to bring a lot of patience.
