For residents of Metro Manila and nearby areas, please call 023-4913479 during clinic hours, 2-7pm, to book your Grab/Lalamove etc. Kindly provide the following details:

1. Name of recipient
2. Contact number of recipient
3. Choice of pickup at LBC branch (specify which) or at home (indicate home address)
4. UIC (XX-YY-00-mmddyyyy)
Where, XX is first 2 letters of mothers name, YY is first 2 letters of fathers name, 00 is birth order, and mmddyyyy is the birth date.

6. missed pills in the last 30 days
7. remaining pill count
